I would always recommend responding with ‘prefer not to say’ on all forms until after interview, so that the company are not taking any disability into consideration. After interview and acceptance, answer with as much detail as possible if you require any support, for example, if you require a specially designed chair, or if you require medication while at work, for example Type 1 Diabetes.
